Board Contact Form

This form generates a secure and confidential message which is sent directly to the Executive Board’s email inbox, and is intended for the following purposes.

  1. To submit topics for Board review, such as for general awareness and discussion, or for a proposed policy update.
  2. To express interest in serving on the Board or a Board committee.
  3. To request that an item be added to the Board Meeting Agenda. (NOTE – Agenda topics must be submitted by the 1st Friday of the month.)
  4. To report suspected violations as defined in CMA’s Whistleblower Policy.  (NOTE – Your name and email address will be necessary for follow up purposes.)
